If you’re a team captain or sponsor lead, here’s what we need to finalize your team registration:

Please submit the following for each of your four players:

1. Full Name (first and last)

2. GHIN or IGA Number (if applicable)

3. State where GHIN/IGA is located

4. Email Address

5. Cell Phone Number

6. Date of Birth

7. Gender (to assign appropriate tees)

8. Club or Company Affiliation

If a player doesn’t have an official handicap, we’ll use their contact info to follow up via Golf Genius to gather that detail directly.
